It gets between the bricks
Ivy stems look harmless on the face of a wall, but once they find a gap in old or soft mortar they grow into it. As the stems thicken they push the bricks apart from the inside, which is what was happening on Todd's wall.

It can. On sound brickwork with good mortar, ivy mostly sits on the surface. But where the mortar is old or soft, the stems grow into the joints and thicken, pushing bricks apart, which is what had happened here.
Yes, once it gets into the joints. As the stems grow between the bricks they loosen them and open up the mortar, which over time weakens the wall. Todd's wall was already showing serious structural problems.
